Before You Ship Your Sample
Important steps:
Confirm that the Sample ID shown in the form matches the Sample ID printed on your Sample Return Tube.
Complete all required fields.
Collect your sample according to the kit instructions.
Place the Sample Return Tube into the Specimen Transport Bag.
Pack the sample with the frozen ice pack in the Insulated Shipping Container (Styrofoam Box).
Ship your sample as soon as possible using a 2-day shipping service.
Important: Samples collected more than 5 days before receipt by the laboratory may not be processed.
